Indications for use
- Cryptococcosis, including cryptococcal meningitis and other localizations of this infection (including lungs, skin), in patients with a normal immune response and in patients with various forms of immunosuppression (including AIDS patients, during organ transplantation). Fluconazole can be used to prevent cryptococcal infection in patients with AIDS;
- Generalized candidiasis, including disseminated candidiasis, candidaemia and other types of invasive candidiasis infections (infections of the eyes, endocardium, Geniel, urinary and respiratory tract). Fluconazole can be prescribed to patients with malignant neoplasms, patients in intensive care units, patients undergoing immunosuppressive or cytostatic therapy, as well as in the presence of other factors predisposing to the development of candidiasis;
- Candidiasis balanitis;
- Candiduria, non-invasive candidiasis of the skin, bronchopulmonary candidiasis, candidiasis of the mucous membranes, including the esophagus and the pharynx and oral cavity (atrophic candidiasis of the oral cavity caused by wearing dentures); Fluconazole is also used to prevent relapse of oropharyngeal candidiasis in patients with AIDS;
- Genital candidiasis: vaginal candidiasis (chronic recurrent and acute), prevention to reduce the recurrence rate of vaginal candidiasis (from 3 episodes per year);
- Fungal infections in patients with malignant neoplasms who are predisposed to such infections due to radiation therapy or chemotherapy with cytostatics (prevention);
- Multi-colored (pityriasis) versicolor;

- Candidiasis of the skin;
- Onychomycosis;
- Deep endemic mycoses, including histoplasmosis and coccidiomycosis in patients with normal immunity.
Contraindications
- Simultaneous use with astemizole or terfenadine (against the background of constant therapy with fluconazole at a dose of 400 mg per day), as well as other drugs that extend the QT interval;
- Age up to 4 years (for capsules);
- Lactation period (for infusion solution);
- Hypersensitivity to the components of the drug, as well as to other azole antifungal drugs in history.
Fluconazole is prescribed with caution in the following conditions / diseases:
- Liver failure;
- Renal failure (for solution for infusion);
- The appearance during the treatment of a rash in patients with superficial fungal infection and systemic / invasive fungal infections;

Dosage and administration
The daily dose of fluconazole is determined by the nature and severity of the fungal infection. Change the daily dose when switching from intravenous administration to taking the drug inside, and vice versa, there is no need.
Fluconazole in the form of capsules and tablets is taken orally, the solution for infusion is administered intravenously drip at a rate of not more than 200 mg per hour.
The infusion solution is compatible with Ringer's solution, 20% dextrose solution, 4.2% sodium bicarbonate solution, Hartman solution, 0.9% sodium chloride solution, 0.9% potassium chloride solution and 5% dextrose solution. Infusions can be carried out using conventional transfusion kits using one of the above solvents.
The dosage regimen is determined by simple indications.
With cryptococcal meningitis and cryptococcal infections of other localizations, adults and children from 15 years old (with a body weight of more than 50 kg) are prescribed 400 mg on the first day, then 200-400 mg once a day. The duration of therapy for cryptococcal meningitis is at least 6-8 weeks, for cryptococcal infections, the course duration is determined by the clinical effectiveness, confirmed by the results of mycological studies.
In patients with AIDS, to prevent the relapse of cryptococcal meningitis, fluconazole is prescribed after completing the full course of primary therapy at 200 mg per day for a long period of time.
In the treatment of disseminated candidiasis, candidaemia, and other invasive candida infections, Fluconazole should be taken at a dose of 400 mg on the first day, then the dose should be reduced by 2 times (with insufficient clinical effectiveness, the dose should not be reduced). The duration of the course is determined by the clinical effectiveness of the drug.
In the treatment of deep endemic mycoses, the use of fluconazole in a daily dose of 200-400 mg for up to 2 years may be required. The duration of the course is determined individually: coccidioidomycosis - 11-24 months, histoplasmosis - 3-17 months.
With a creatinine clearance of 11-50 ml per minute, 50% of the recommended dose of fluconazole in capsule form should be used or the interval prescription drugs doses of the usual dose should be increased (1 time in 2 days). As a rule, in patients with functional renal impairment, the drug is prescribed depending on the creatinine clearance as follows (dose / interval between doses of the drug):
- From 40 ml per minute: usual daily dose / 24 hours;
- 21-40 ml per minute: usual daily dose / 48 hours or 1 /2 daily dose / 24 hours;
- 10-20 ml per minute: usual daily dose / 72 hours or 1 /3 daily dose / 24 hours.
Patients on hemodialysis should take one dose of fluconazole after each session.
The daily dose of fluconazole in the form of a solution for infusion in children should not exceed 400 mg per day. The drug is prescribed for infections similar Geniel adult ones, the duration of the course depends on the mycological and clinical effect.
The recommended dose in children in the treatment of cryptococcal meningitis and cryptococcal infections of other locations, as well as with generalized candidiasis, is 6-12 mg / kg per day (determined by the severity of the disease).
