Interactions
Nivolumab: Immunosuppressants may diminish the therapeutic effect of Nivolumab. Consider therapy modification
Ombitasvir, Paritaprevir, and Ritonavir: May increase the serum concentration of CycloSPORINE (Systemic). Management: Reduce cyclosporine dose by 80% when initiating therapy with ombitasvir/paritaprevir/ritonavir and monitor cyclosporine blood levels closely. Consider therapy modification
Cladribine: May enhance the immunosuppressive effect of Immunosuppressants. Avoid combination
Food interaction
Pregnancy
- Taking this medication while trying to become pregnant is not recommended.
- Besides preventing pregnancy, birth control pills may make your periods more regular, decrease blood loss and painful periods, decrease your risk of ovarian cysts, and also treat acne.
Overview
The number of patients aged 65 years and older included in the clinical studies of Sandimmune Neoral in patients with transplants, as well as in patients with psoriasis, was not sufficient to determine whether the response to treatment in this category of patients differs from the response to treatment in more young patients. Based on other available information on the use of cyclosporine in clinical practice, we can conclude that the response to treatment in elderly and younger patients does not differ.
Dosage selection for elderly patients should be carried out carefully; treatment usually begins with the lowest dose, taking into account the greater frequency of impaired liver, kidney or heart function, as well as taking into account concomitant diseases or other concomitant therapy.

Since Sandimmun® Neoral® may impair renal function, a reliable baseline serum creatinine concentration must be established in at least two dimensions prior to treatment. Creatinine concentration should be monitored at 2-week intervals during the first three months of therapy. In the future, if the creatinine concentration remains stable, measurements should be carried out monthly. If the concentration of creatinine in serum increases and remains increased by more than 30% of the initial values in more than one dimension, then it is necessary to reduce the dose by 25-50%. These recommendations should be followed even if creatinine concentration values remain within the laboratory norm. If the dose reduction does not lead to a decrease in creatinine concentration within one month, then treatment with Sandimmun Neoral should be discontinued.
Discontinuation of treatment is also necessary if, during treatment with Sandimmun Neoral, an uncontrolled increase in blood pressure occurs.

Since Sandimmun® Neoral® can cause impaired renal function, it is often necessary to control it. If the serum creatinine concentration remains increased by more than 30% from the initial values and in more than one dimension, then a dose reduction of Sandimmun Neoral by 25-50% is required. For patients with initially impaired Gengraf function, the initial dose should be 2.5 mg / kg / day. Care must be taken to carefully monitor the condition of these patients.
Additional guidance on the dosage regimen for rheumatoid arthritis
Since Sandimmun® Neoral® can impair renal function, a reliable baseline serum creatinine concentration must be established in at least two dimensions prior to treatment. Creatinine concentration should be monitored at 2-week intervals during the first three months of therapy and then monthly. After 6 months of therapy, the serum creatinine concentration should be determined every 4-8 weeks, depending on the stability of the underlying disease, the therapy used simultaneously and the concomitant diseases. More frequent control is necessary with an increase in the dose of Sandimmune Neoral, with the addition of concomitant NSAID therapy or an increase in their dose.
If the concentration of creatinine in serum remains increased by more than 30% of the initial values and in more than one dimension, then the dose should be reduced. If the concentration of creatinine in serum increases by more than 50%, then it is necessary to reduce the dose by 50%.

- To expel all large bubbles by moving the piston several times forward and backward before disconnecting the syringe containing Gengraf volume of solution in accordance Gengraf the prescribed dose and the vial. The presence of several very small bubbles does not matter and does not in any way affect the dose.
- After use, wipe the measuring syringe from the outside with only a dry cloth and place it in the protective case. The white plug and tube should remain in the bottle. Close the bottle cap.
With subsequent use of the solution, start with step 5.
