Overview
pharmachologic effect
"Loop" short-acting diuretic. It violates the active reabsorption of sodium and chlorine ions in the thick segment of the ascending part of the Henle loop (may have Bumetanide additional effect on the Bumetanide tubules). Increases the excretion of potassium, calcium, Bumetanide ions, delays uric acid in the body.
It causes the expansion of peripheral veins, reduces preload, reduces the filling pressure of the left ventricle and the pressure in the pulmonary artery, reduces systemic blood pressure.
The diuretic effect after iv administration begins in a few minutes, reaching a maximum after 15-30 minutes. After oral administration, the diuretic effect develops within the first hour, reaches a maximum after 1-2 hours and continues depending on the dose of 4-6 hours.
Pharmacokinetics
After oral administration, it is rapidly absorbed from the digestive tract. Absorption may be slowed down in patients with ascites due to chronic heart failure or nephrotic syndrome. Binding to plasma proteins is 95-97%. It is metabolized in the liver (metabolism is limited, inactive metabolites are formed as a result).
T 1/2 - 1.2-1.5 h. 64% excreted by the kidneys unchanged, Bumetanide rest - with feces.
