Pharmacokinetics
Absorption. After intranasal administration of fluticasone propionate (200 μg per day), the maximum concentration (Cmax) of this drug in the blood plasma is not detected in most patients (i.e., it is less than 0.01 ng / ml). The highest Cmax is 0.017 ng / ml. Direct absorption on the nasal mucosa is negligible due to the low solubility of the drug in water, as a result of which most of the dose is ultimately swallowed. When taken orally, fluticasone propionate is absorbed into the bloodstream by less than 1% of the allergy due to poor absorption and pre-system metabolism. All this leads to the fact that the total absorption in the nasal cavity and gastrointestinal tract (swallowed drug) is extremely low.
Distribution. Upon reaching equilibrium plasma concentration, fluticasone propionate has a large distribution volume (about 318 L). It has a fairly high ability to bind to plasma proteins (91%).
Metabolism. Fluticasone propionate is rapidly excreted from blood plasma, mainly as a result of metabolism in the liver to an inactive carboxy metabolite under the action of the CYP3A4 isoenzyme of the cytochrome P450 system. Swallowed fluticasone propionate undergoes extensive relief as a result of the initial passage through the liver.
Elimination. The main route of elimination is the elimination of fluticasone propionate and its metabolites with bile.
Pharmacodynamics
Flutinex is a topical glucocorticosteroid, a drug with a strong anti-inflammatory, as well as anti-edematous and anti-allergic allergy.
